Overview and Instructions

 

Overview

 

The University of Oregon has created a website (www.pbssurveys.org) for the online completion, tabulation and report review of three different types of surveys that may be used by SW-PBS teams to assess school climate and team function.  The three survey types are:

 

  • School Safety
  • Self Assessment
  • Team Checklist

 

The address for the website is www.pbssurveys.org.  More information about each survey type is found by placing the cursor over the “Surveys” button along the top navigation bar and selecting the desired survey.  For each survey type, there are also options to “Complete Survey” and “View Reports”.  Clicking either of these options will bring you to a login screen.  Each school has been assigned a unique login number that can be used by any individuals in the school community to complete specific surveys and view the report after the survey “end date” passes.  Your school’s survey code should be kept in your team notebook for easy reference.  If you should need to inquire about your number, you may contact Susanna or Betty at any time for the information.

 

Set-Up

 

Important:   Please use your calendar to know when the survey windows will be open for your use.  If you should accidentally miss the window, the internal coach or principal must contact Betty or Susanna with the following information:

  • The type of survey the school/team wishes to complete
  • The desired start date (when the survey will be available for individuals to complete on-line)
  • The desired end date (when the survey will “close” so that analysis of results can begin)

 

The set-up of the survey can be done very quickly – usually on the same day the request is made. 

 

Survey Completion

 

Once the survey is open, expected participants can be invited via written instruction or email to complete the survey on-line.  (If you email the instructions, people may link directly to the survey website.) The instructions to participants will need to include the following information:

 

To log in and enter surveys, go to the website www.pbssurveys.org.   Along the top navigation bar, you will see a choice for “Surveys”.  When you put the cursor over this, various survey types will pop up – choose the one titled “(put the survey name here)” and click “Complete Survey”. This will bring you to the login screen.  Leave the type of login at “Respondent” and type in the School Account code, (which is (put the login code here) for (put school name here)), and click Login. This will bring up the survey itself.  Complete the questions on the survey.  When you reach the end, simply click the “Submit” button, and you’re done! Depending on the type of survey, you may or may not be required to answer each question.  On the Self-Assessment survey, for instance, people can leave certain questions or whole sections blank if they have no basis on which to answer.  Please note that you must complete the survey before (closing date).  After this date, the survey closes, no more surveys are accepted and the analysis of results becomes available.

 

Viewing Results/Reports

 

After the end date and the close of the survey, respondents may login again at any time to see the results of the survey.  They should login the same way they did before, but this time, they should select “View Reports” instead of Complete Survey.  Additional detail may be available on some reports by clicking the “+” sign.  Clicking on the “Show Charts” or “Show Graphs” buttons will present information visually.
